Professional Career:
Dr. Philip Augustine started his clinical practice after post graduation in Medicine from the AIIMS, New Delhi in a rural set up at the Deva Matha Hospital, Koothattukulam, Ernakulam District, Kerala. With the sound background in clinical and academic medicine from the Kottayam Medical College and AIIMS, New Delhi I could establish a popular and scientific medical practice in this rural area of Kerala, in my capacity of Medical Director & Chief of Medicine. In the 4½ years of work there I could bring up the 75-bedded general hospital to a 350-bed facility with all basic facilities needed for the 50,000 population in the area. Also major medical problems were referred to the Deva Matha Hospital during that period. During this stint as a Physician gave me an insight into the magnitude of the Gastrointestinal diseases in Kerala and hence convinced me the need for further specialization in Gastroenterology. This was a period (the 1970’s & 80’s) when technological advances in Medicine in the form of endoscopy, ultrasound scan and computerized tomography (CT) scan gave a major fillip to the scientific practice of Gastroenterology. Hence I decided to specialize in Gastroenterology and joined the prestigious PGIMER, Chandigarh for the DM course in 1980. After completing the 2-year postdoctoral programme I returned to the same institution in Koothattukulam in 1982 and established the first private sector Gastroenterology unit in Kerala. In the next 10 years till 1992 I could bring up a comprehensive Gastroenterology unit at the Deva Matha Hospital with a major endoscopy and surgical set up. The small town of Koothattukulam found a place in the Gastroenterology world literature by our publications in Tropical Pancreatitis – an enigmatic medical problem with the highest world incidence in Kerala. With the help of the Welcomme Trust, London and in association with the Endocrinology Department of All India Institute of Medical Sciences, a door to door survey of tropical pancreatitis and diabetes in Kerala was conducted by my team in the Bharananganam Panchayat of Meenachil Taluk, Kottayam District. A prevalence of tropical pancreatitis of 4 per 1000 and an incidence of diabetes of 4% in rural Kerala were detected in the survey. We also reported for the first time in India the occurrence of Recurrent Pyogenic Cholangites (RPC) also known as Oriental Cholangiopathy. While in Koothattukulam, I could also spread the message of modern Gastroenterology practice through out the length and breadth of Kerala through the various Indian Medical Association (IMA) and Association of Physicians of India (API) state forums. I held official positions in IMA as the Muvattupuzha Branch President and in API as its state Vice President during this time.
Sine the scope of my clinical and research work outgrew the small town of Koothattukulam, in 1992 I moved into the city of Cochin with my team of Gastroenterologists and established the Digestive Diseases Centre at the newly formed PVS Memorial Hospital. The next 10 years saw the rapid growth of this unique centre of Gastroenterology where patients were referred from all across the State and from other States in India for major gastrointestinal problems. It was at PVS Memorial Hospital where I convincingly reported the existence of Crohn’s disease in India at the Indian Society of Gastroenterology national conference in 1995. This was an enlightening revelation to the entire Gastroenterology community in India and now it is an accepted fact that Crohn’s disease also has possibly the highest incidence in Kerala among the Indian States. Many national and international Gastroenterologists came for advanced training in Gastrointestinal Endoscopy to PVS Memorial Hospital as I had already established a pioneering position in various advanced therapeutic endoscopic procedures including ERCP and laser endoscopy. We could also start the DNB (Diplomate of the National Board) programme in Gastroenterology at PVS Memorial Hospital. Our infrastructure and confidence were good enough for me to organize the 43rd National Conference of the Indian Society of Gastroenterology in 2002 at Cochin. This was acclaimed by all as one of the best national conferences in the history of ISG.
To further the standard of referral tertiary care in Gastroenterology and other branches of medicine I took the initiative of establishing the now well known Lakeshore Hospital & Research Centre, Cochin, a 350-bed multispeciality hospital with 36 clinical departments, over 130 doctors and a total of over 1200 employees in the corporate sector in Kerala. I could attract the best professional medical talent in the state and organized a new business model making senior medical specialists as the nucleus of the venture making them stakeholders in the public limited company. This model is now being emulated by various groups of doctors across the country. As a medical institution excelling in patient care, education and research Lakeshore in its short history of 5 years has already become a brand name in healthcare in the country. This institution has also a leading role in attracting patients for high end medical and surgical treatments from several other countries thus promoting the concept of “Medical Tourism” in the country. Medical Tourism is expected to bring in major foreign currency earnings for the country in the coming years. In the capacity of Chairman Healthcare Subcommittee, Confederation of Indian Industry (CII), Kerala Council, I was also instrumental in spreading the concept of medical tourism in Kerala and organized the “Kerala Health Tourism Conferences 2006 and 2007” in association with the Government of Kerala through its Tourism Department and CII Kerala Council. I was the organizing Chairman of the KHT conferences, which were international conferences with exhibition.
Lakeshore Hospital is one of the few major hospitals in the country which has won the prestigious NABH (National Accreditation Board for Hospitals and Healthcare providers) accreditation. We are recognized for DNB training in various departments including Family Medicine, Gastroenterology, Gastrointestinal Surgery and Anesthesiology. We also run job oriented courses as part of our social commitment for skill development for the educated unemployed. These courses are run under the auspicious of agencies like the Bharat Sevak Samaj and the Christian Medical Association of India (CMAI). The first batch of students have already passed out in Endoscopic Technology, Dialysis Technology and Operation Theatre Technology.
I have also initiated a comprehensive health survey of the Karimaloor Panchayat in Ernakulam District. This is possibly the first such initiative in the private sector to assess the disease patterns and overall health status of a community. This is intended to be a comprehensive door-to-door survey which should generate hard data for future health care planning of the community.
Research Activities:
I could also concentrate on clinical research pertinent to the problems of Kerala. Tropical Pancreatitis, the enigmatic disease with the highest world incidence in Kerala is one of the key area of our research. We already have several publications in the surgical aspects and cancer risk of this disease. I was fortunate to attract the attention of Prof. Henri Sarles of Marseilles, France who sent a team to study this problem in comparison with the alcoholic pancreatitis of Europe. A landmark publication resulted in 1994 in the “Digestive Diseases and Sciences”. We also reported in 1992 in the “American Journal of Gastroenterology” the increased risk of cancer of pancreas in patients with tropical pancreatitis. We are now concentrating on the genetic aspects of this disease to see if an aetiology can be found.
A few publications also have resulted in the field of therapeutic endoscopy, portal hypertension and other liver diseases. A molecular laboratory is almost nearing completion in Lakeshore which should give a fillip to our basic research programmes. We also work in collaboration with the Cochin University of Science & Technology (CUSAT) in research activities especially in Biotechnology. I was a member of the faculty of Medicine at CUSAT. I also function as the Vice President of the Biotechnology Society of India. I am also the Managing Trustee of the Lakeshore Medical Research Foundation, which runs the Lakeshore Medical Library and organizes other research programmes of the institution.
